The Assurance Wireless Lifeline Phone Program

Provides a free cell phone and 215 minutes per month for a year to allow qualified individuals a worry-free way to stay connected.  The person applying needs to be on either medical, SSI, disability, etc. to qualify.  You can go to their website at www.assurancewireless.com to get the application.  Their address is P.
